Discharged from the hospital, now what?

Before you are allowed to return home after your stay, there are a number of things that need to be put in order. In this brochure you will find the main priorities.

If you have questions about your follow-up care, the nurses will be happy to answer your questions. You can also contact the social service.

Discharge

The time of discharge will be discussed with you. Be sure to arrange for your transportation home. If this is a problem, inform the nurses as soon as possible.

Follow-up care

To arrange for your follow-up care, you will be given the necessary information by the nurses. The attending physician will also give you a letter for your GP.

Make sure you know which medication you have to take. Don't hesitate to ask the nurses or physician again.

Do you need a follow-up appointment ? We will schedule this in advance, during your stay or we will ask for your contact information.

If you need support at home, then it is best to notify the home care services in time. For all your questions about home care, you can contact social services.
